Insights The NewYork-Presbyterian Regional Health Collaborative uses an integrated network of patient-centered medical homes to form a “medical village,” linking patients, providers and community-based resources. Developed by New York Presbyterian Hospital, Columbia University and several independent physician organizations, the collaborative developed as an innovative approach to supporting the health of a specific patient population. The organization provides coordinated care through focusing upon advanced health IT (EHR, disease registries) and incorporating cultural competency relating to the largely Hispanic population being served. One recent study shows the program’s early success in reducing ED visits.
As organizations in all parts of the country continue to experiment with PCMH delivery, early gains by programs such as these should be collected, examined and where appropriate, incorporated into planning for this reform-driven care model. |
